  1. You've missed the entry deadline, Before you fill out this form, find out whether we will permit your entry at all: visit the Students & Teachers page and follow the late entry procedures there.
  2. Read the Students & Teachers page and the EISEF Privacy Policy. Plan and conduct your experiment�this includes filling out and signing all the certificates you're supposed to, when you're supposed to.
  3. Collect all the information you need before you start to fill out this form. You may want to print this page so you can refer to it while you collect the info.

  6. Teachers: Many of you fill out the form for all your students. To help you with this, we've added the Clear exhibit and student data button, which does what the name says, but leaves the adult sponsor and school information intact. After you've submitted one entry, click this button before you start the next�it will help keep you from carrying over one project's data to the next project by mistake.
  7. Carefully proofread your typing. What you enter on this form is what will appear on your certificates, in the fair booklet, and on our web site, where people can read it for many years. Also, if the phone numbers or e-mail addresses are wrong, we won't be able to contact you or your adult sponsor about any problems with your entry.
  8. After you finish filling out the form, click the Submit Entry to EISEF button (the one right here or at the bottom of the page). This will e-mail your entry's information to EISEF and will display a signature page with your information. (You have now submitted your entry. In a few days it will appear on the Student Entry Status page, with a status of Pending.)

  9. Print all the sheets of the signature page when it appears.
  10. All the student experimenters and your adult sponsor must sign the printed copy of the signature page.
  11. Mail or e-mail all the sheets of the signed signature page to one of the EISEF addresses given at the bottom of that page; include copies of all the other required documents and certificates. (When we receive your paperwork and make sure that everything is in order, we�ll change your entry�s status to Accepted. We�ll contact you or your adult sponsor if everything isn�t in order.)
