Instructions for requesting an EISEF background check

1. This background check will cover only the 2024 EISEF season. You need a new check every year.
2. Read this whole page, including the Affirmation, before you start to fill out the form.
3. Fill out the form and proofread your typing.
4. After you finish filling out the form, click the Send to EISEF button (the one right here or at the bottom of the page). This will email your information to us and display a confirmation page.
   
5. You have now submitted your request. In a few days it will appear on the Background Check Status page, with a status of Pending or Cleared.

Affirmation

I hereby state and affirm:

1. I am 18 years old or older.
2. I acknowledge that my EISEF background checks for previous years are no longer valid for the current 2024 EISEF season.
3. I acknowledge that I am volunteering for a position with direct access to minors. Therefore, I consent to a background check of criminal and sex offender records in any U.S. state or territory I have lived in for the past five years, including (but not limited to):
   1. National Sex Offender Public Website
4. I shall fully cooperate in the process, including ensuring the correct identity has been checked.
   1. On request, I shall confirm my identity with a government-issued photo ID that matches the information I have provided.
5. I acknowledge that, if I fail the background check, the Board may at its discretion order me to to cease volunteer work for EISEF.
   1. If I have been ordered to cease working for EISEF, I will avoid locations hosting EISEF functions, such as the Fair itself; and I will cease to claim that I am working for EISEF in any capacity.
6. I agree that I shall, at no time while volunteering for EISEF, have one-on-one contact in private with any minor children who are not my legal children. I shall at all times have at least one additional adult within my presence when in private with a minor child.
7. I shall, at no time under the auspices of any EISEF activity, ask a minor child to accompany me to a private location, take any minor child in my private car, or share contact information with a minor child.
